NAME CHANGE CHECKLIST
Here's a list of places that you should remember to contact if you decide to change your name after the wedding:
When changing your last name, you will first need to obtain a certified copy of your marriage certificate from the County Recorder at the same place where you pick up your marriage license. It is usually available about two weeks after your wedding ceremony. Here is the Aplication Form. Some agencies will require a copy of the certified license. Remember that the marriage certificate will not be automatically sent to you. You must request it.
Once you have the certificate, you must take it to the Social Security office to change your name with them. After that, you need to change it at the DMV. Once you receive the new social security card and dricer license in the mail, you may procees with contacting all the businesses and government agencies that still have your maiden name.

Social Security Card – Application Form and instructions for a new Social Security Card (SS-5 Form)
Driver License - (Don't forget to also update your vehicle registration, address change and voter registration while at the DMV)
IRS - Once you change your name with the Social Security Administration, the IRS will be informed within 10 days. So no additional work is necessary on your part.

Friends & Family (Notify them when sending the thank you cards)

American Bar Association
Power of Attorney
American Red Cross or Blood Centers of the Pacific
National Marrow Donor Program
You may list your name as Jane (Smith) Jones to show that your old company may have you listed under a different last names.
